As COVID-19 cases surge uncontrolled in the United States, the specific conditions of ICE detention place&nbsp;<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.npr.org_2020_07_01_871625210_you-2Dcan-2Deither-2Dbe-2Da-2Dsurvivor-2Dor-2Ddie-2Dcovid-2D19-2Dcases-2Dsurge-2Din-2Dice-2Ddetention&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=r2NxKgyPrA_VZXL-5huP8cLV_xOj6mSSslEJuHGfsNg&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">detainees at a higher risk of exposure<\/a>. Of 310 reported tuberculosis (<a rel=\"noreferrer noopener\" href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.cdc.gov_tb_statistics_reports_2018_table37.htm&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=RFVju8nsgcjJencU_Zk-9QQapc8NdG1WSodxmL4UHts&amp;e=\" target=\"_blank\">TB) cases in U.S. carceral facilities<\/a>\u00a0in 2018, nearly half were diagnosed among ICE detainees.\u00a0 Like COVID-19, TB is a highly contagious respiratory illness that spreads quickly in confined areas, putting<a rel=\"noreferrer noopener\" href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__pubmed.ncbi.nlm.nih.gov_17572305_-23-3A-7E-3Atext-3DBACKGROUND-253A-2520People-2520detained-2520by-2520United-2CImmigration-2520Health-2520Services-2520-28DIHS-29.&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=C4nzSgqWtpTj5K-vLwpy2UoMk6yQLn8Nad3kxN-nQgY&amp;e=\" target=\"_blank\">\u00a0detainees at risk<\/a>.<a rel=\"noreferrer noopener\" href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.propublica.org_article_ice-2Dhas-2Drepeatedly-2Dfailed-2Dto-2Dcontain-2Dcontagious-2Ddiseases-2Dour-2Danalysis-2Dshows-2Dits-2Da-2Ddanger-2Dto-2Dthe-2Dpublic&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=di3HvIgS62EMzC203qlt8Lbjpb1LLcKkloP2YpfdsFQ&amp;e=\" target=\"_blank\">\u00a0\u00a0Insufficient responses to TB<\/a>,<a rel=\"noreferrer noopener\" href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0749379707001900\" target=\"_blank\">\u00a0widespread in contracted facilities<\/a>, foreshadowed how ICE would insufficiently respond to COVID-19 as an evolving threat.<\/p>\n\n\n\n<p id=\"_ftnref2\">There is widespread consensus in public health and the social sciences that<a rel=\"noreferrer noopener\" href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S014067361730003X\" target=\"_blank\">\u00a0structural conditions cause interactions between multiple illnesses<\/a>, in turn worsening suffering. When this happens for a particular population due to marginalization, policy, or a social factor, medical anthropologists call it a\u00a0<a href=\"https:\/\/www.tandfonline.com\/doi\/abs\/10.1080\/17441692.2020.1724317\"><em>syndemic<\/em><\/a>.\u00a0Detention centers are a particularly grave risk environment for syndemics. For example, the structural conditions of ICE detention promote multidirectional biological interactions between malnutrition, Type II diabetes, and TB<a href=\"#_ftn2\" data-type=\"internal\" data-id=\"#_ftn2\">[2]<\/a>. In this syndemic model, the structural-biological-biological interactions produced in ICE detention as a result of U.S. government policy exacerbate each illness and increase risks for wider communities.<\/p>\n\n\n\n<p>ICE detention now also produces a syndemic risk environment for<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__definingmomentscanada.ca_news_syndemics-2Dand-2Dhealth_&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=cntxCDoLg67l87MWGSXatU5K1V0OAV_ZWt0wvbGn1Ac&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;COVID-19 to interact with other illnesses<\/a>. In&nbsp;unsanitary facilities&nbsp;where&nbsp;<a href=\"https:\/\/immigrantjustice.org\/sites\/default\/files\/content-type\/research-item\/documents\/2018-06\/Code%20Red-%20detention%20deaths%20report%20June%202018.pdf\">health care is inadequate<\/a>&nbsp;and<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.reuters.com_article_us-2Dhealth-2Dmigrant-2Dchildren-2DidUSKBN1Y72NV&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=D1e_SrM7Pzbo3ANOnWCvNPGC90HYa8od5m3M9UYVnwQ&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;medications (including inhalers) are confiscated<\/a>, rapidly spreading&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S009167492030806X\">COVID-19 is likely to interact with and worsen other respiratory illnesses, such as asthma<\/a>.<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.cdc.gov_coronavirus_2019-2Dncov_need-2Dextra-2Dprecautions_asthma.html&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=0bmiuTfHJnhHVpPUsDx-Ym-mOirkccGx9SNlwVqXoxc&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;Asthma is itself a risk factor for COVID-19<\/a>. When someone with asthma contracts COVID-19, there is also greater risk for&nbsp;<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.cmaj.ca_content_192_20_E551&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=UZRT8imF_eHW0f4L1TZQUAduMSLq6pvZPHJRiYdkZZs&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">progression to severe acute respiratory disease<\/a>&nbsp;(SARD), and<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.sciencedirect.com_science_article_pii_S0012369220307753&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=QIgBvrOvssBCYkq_q3ILIz5Qigv5zuLr4N1uwUTb6GM&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;mortality<\/a>.&nbsp;There is a pressing need for research on the potential for asthma\/COVID-19 syndemics produced by the conditions of ICE detention.<\/p>\n\n\n\n<p>All such illness interactions are particularly urgent to evaluate in light of COVID-19 spreading rapidly in detention facilities. By late March,<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.nytimes.com_2020_03_29_us_coronavirus-2Dmigrant-2Dchildren-2Ddetention-2Dflores.html&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=T7H0wFSEUJtJ1HSAqnWAfSNXtHk-exoHjhu9bzHJYkw&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;a federal judge ordered agencies operating child detention facilities to account for their efforts to release children<\/a>, citing COVID-19 risks after four children tested positive at a New York facility. In early May,<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.npr.org_sections_coronavirus-2Dlive-2Dupdates_2020_05_07_852475822_first-2Ddeath-2Dof-2Ddetainee-2Din-2Dan-2Dice-2Ddetention-2Dcenter-2Dfrom-2Dcovid-2D196_first-2Ddeath-2Dof-2Ddetainee-2Din-2Dan-2Dice-2Ddetention-2Dcenter-2Dfrom-2Dcovid-2D19.html&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=BM2soqFk-rp_UIztyI46zeIL1wdQBqz-M3awnzOtMjk&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;the first confirmed detainee death from COVID-19<\/a>&nbsp;was that of an individual who petitioned for release after other detainees in the facility tested positive.&nbsp;As we submit this, ICE had confirmed more than 5,000<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.ice.gov_coronavirus-23citations&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=iQ2cDo-EK7IMHr-SVGmtaC_81KBH-8SJl3omwix2NMM&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;&nbsp;COVID-19 cases among detainees<\/a>&nbsp;\u2013 double the number of cases from when we began drafting this piece just a few months ago. While the CDC did issue&nbsp;<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.ice.gov_coronavirus-23citations&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=iQ2cDo-EK7IMHr-SVGmtaC_81KBH-8SJl3omwix2NMM&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">guidance for addressing COVID-19<\/a>&nbsp;in carceral settings, ICE\u2019s&nbsp;implementation appears uneven and insufficient. Government-issued health guidelines for detention centers are<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.washingtonpost.com_politics_2019_02_26_did-2Dice-2Dignore-2Dhealth-2Dsafety-2Dstandards-2Djailed-2Dimmigrants_&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=U1aoVzW65NS7ogaklJJ-8GPAAGF41UIEo31Yo4Hf-yE&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;not enforceable<\/a>&nbsp;and<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__abcnews.go.com_Politics_doctor-2Dadvised-2Dhomeland-2Dsecurity-2Dtestifies-2Dcovid-2D19-2Dprotocols_story-3Fid-3D71030467&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=ZQ3fIx5JgsO3ytjTPqKwu_cMmsSSMSTMcYR9goe4RaI&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;many centers choose not to comply<\/a>.<\/p>\n\n\n\n<p>ICE<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__www.nytimes.com_2020_05_15_nyregion_coronavirus-2Dice-2Ddetainees-2Dimmigrants.html&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=4kOT4WhsuUw4DhrH3GrkOEYsIKEpkzJFxPxGfAjot8Y&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;released only some 900 individuals from mid-March<\/a>&nbsp;to late August, a grossly inadequate response to address the syndemic conditions of detention. Over 4,000 physicians<a href=\"https:\/\/urldefense.proofpoint.com\/v2\/url?u=https-3A__docs.google.com_document_d_1eNyNmy-2D622OjVILFSwgypITPK0eAt5yLgSkS-5F7-5F0vv8_edit&amp;d=DwMFaQ&amp;c=iORugZls2LlYyCAZRB3XLg&amp;r=gzQb6ShGS_-cNrn594rNq73nkMvfVI7aob5AY35iEYk&amp;m=JMmqzTpLRtPx45KBQslY3Pj9DDirWpGgjT5ejbdDahE&amp;s=fcvNs3rrti4-rEU30u0s5D5C_y-GJzscVFn4PTEEmx8&amp;e=\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;signed an open letter<\/a>&nbsp;demanding ICE release detainees with health conditions that may interact with COVID-19.
